This position will pay up to $31.

What You Will Do Gathers information from participants and/or their family members about health, home environment and individual needs to share with In-Home Services RN, who then performs initial assessment, semi-annual reassessment, annual reassessment, and change of condition assessments of participants in the their respective home environments.
Provides skilled supportive and restorative nursing care to participants as ordered, and educates participants in their homes about health care.
Assists RN or Manager with the coordination of each participant's plan of care by initiating contact and services to be provided with appropriate community resources and Interdisciplinary Team (IDT).
Maintains the participant's medical record with thorough documentation.
Skills & Requirements Current Licensed Practical Nurses License, in the respective state in which he/she is employed.
One (1) year of nursing experience in a hospital, nursing home or community based setting; home health experience preferred.
One (1) year of experience working with the frail or elderly population required, or completion of job specific training related to working with the elderly population must be completed within the first six months of hire.
Must have a valid driver's license, proof of insurance and have means of transportation.
